Order & pick up attendance and ADAP forms for driving tests this summer

Many students will be taking driving tests this summer to obtain their licenses. However, students need to bring two school forms to the test: the attendance form and the ADAP form.

Both forms need to be ordered at the front office either with Ms. Powell or Ms. Buckner. Students must bring two dollars to cover the notarization fee on the certificate of attendance. Usually, the forms are ready to be picked up later the same day or the subsequent school day.

Most students took the ADAP course and its test freshman year in health, so the ADAP form simply certifies that a student has successfully completed the course.

The certificate of attendance certifies that a student has not had more than ten unexcused absences and has not committed any serious criminal offense.

Since both forms are mandatory for high school students taking a driving test, make sure to order and pick up these two forms in the front office during this last week of school. If not, you will likely have to wait until school starts again to get your license.
